Thank you for your interest in Clearview Christian School.

    The following steps summarize the enrollment process. The necessary forms are attached. If we can be of further help to you, please feel free to call.

1. Application: Complete the application and return it as soon as possible. We must have a copy of the birth certificate that is recorded in the State Security of Vital Statistics, a copy of the Social Security card, and the medical information that shows each student’s immunizations and boosters are up-to-date. Also, we must have a copy of the latest report card for the students in grades 1-12. (If applying for kindergarten, students must be 5 years old by September 30th.)
    

2. Registration Fee: An application fee of $60.00 plus the $135.00 registration fee for all students PK2-12. These fees are both non-refundable. This includes students who have been away from Clearview Christian School for a semester or more. There will also be a book fee, which is based on grade level. Electives fees are included in the book fees for grades 9-12.
    

3. Interview: An interview will be scheduled to discuss your child’s enrollment after we receive your application form and fee.4.
    

4. Physical Examination: This form should be completed and returned as soon as possible. This MUST include immunization records and be signed by a physician.5.
    

5. Reservation: We will not be able to reserve a place for your child until the following items are completed:
    

   • Application forms and fee

   • Registration fee

   • Financial Agreement

   • Family Agreement

   • Parent and School Partnership Commitment Form

   • Emergency Medical Information Card

   • School Health Examination and Immunization Record (by 1st week of August)

   • Copy of birth certificate and Social Security card* Book fee (by July 14, 2006)
